History and physical examination

Is the weakness and numbness affecting almost all nerves (symetric polyneuropathy)?	Yes	No
Is the weakness or numbness only affecting one nerve (mononeuropathy)?	Yes	No
Is the weakness or numbness affecting two more nerves (eg, both feet effected)(multifocal mononeuropathy)?	Yes    No
