Those with short initial remissions (< 1 y) should receive high-dose chemotherapy with stem cell support, perhaps after a course or two of combination therapy to put the patient in a minimal disease status at the time of high-dose therapy. Effective high-dose regimens include lomustine, cyclophosphamide, and etoposide (CBV) and lomustine, etoposide, cytarabine, and melphalan (BEAM).
