For patients with relapsed disease after initial CR with doxorubicin (Adriamycin), induction chemotherapy followed by ABMT is the current treatment of choice for those with IPI of 1 to 3; however, results for patients with IPI of 0 after standard salvage therapy are similar to those for patients undergoing ABMT. For patients with disease refractory to initial therapy, chemoinsensitive relapse, or those unable to undergo ABMT, remission can be induced with ifosfamide (Ifex), mitoxantrone (Novantrone), and cisplatin (Platinol-AQ) in various combinations. Promising agents in the salvage setting include paclitaxel (Taxol), docetaxel (Taxotere), topotecan (Hycamtin), and gemcitabine (Gemzar).
