Initiate one of the following drugs:
Pentoxifylline (Trental), 400 mg by mouth three times a day reduces blood viscosity by increasing erythrocyte flexibility, inhibiting platelet aggregation, and reducing plasma fibrinogen levels but has had mixed clinical results in the treatment of intermittent claudication.
Cilostazol (Pletal), 100 mg by mouth twice a day, has been approved for the treatment of intermittent claudication.
Treatment of neurotropic diabetic foot ulceration is becaplermin (Regranex) gel. Becaplermin contains recombinant human platelet-derived growth factor and is used topically on the ulcer once a day.
