Patients with severe limb-threatening ischemia as evidenced by rest pain, ulcer, or gangrene should receive vascular surgery consultation and proceed to revascularization.
Endovascular therapy may also be used in the treatment of limb-threatening ischemia. To increase arterial inflow, percutaneous transluminal angioplasty, with or without stent placement, is the treatment of choice for focal iliac stenoses before infrainguinal bypass.
