While ovarian tumors demonstrate tremendous histological diversity, there is considerable overlap in their gross appearance making their distinction often difficult on gross examination alone.
A variety of clinicopathologic data are important to help narrow the diagnostic choices.
Age of the patient
(1) If < 50, only approximately 13% are malignant
(2) If > 50, approximately 45--50% malignant
(3) Dermoid cysts and sex-cord stromal tumors occur at all ovary cancer ages
(4) Malignant germ cell tumors almost always occur in those younger than age 50
b. Bilateral or unilateral tumor
1) Sex-cord stromal tumors are almost always unilateral and
confined to the ovary
(2) Metastatic tumors are bilateral 65 % of the time
